FDA Approves Silicone Gel Implants


In Late November 2006 the FDA approved the silicone gel filled implants for women 22 years old and up. This decision was based on years of research and data collection by both implant manufacturers Inamed and Mentor as well as testimonies from patients and doctors. The studies confirmed repeatedly that there was no association between these implants and connective tissue diseases (Lupus, scleroderma, rheumatoid arthritis) or cancer.
The main advantage of silicone implants are that they feel more natural since their consistency is closer to a female breast as compared to saline filled implants. This is especially advantageous to thin women that don’t have much breast tissue to cover the implants. In addition, there is less chance of rippling and visibility of the implant in these women.

If a gel filled implant ruptures it usually stays inside the pocket until the leakage is discovered and the implant is replaced. If the gel leaks outside the pocket it can create a foreign body reaction where the immune cells try and eat up the silicone particles. The manufacturer’s now have developed better gel filled implants with thicker shells as well as more cohesive silicone gels (some refer to "gummy bear" implants) which have reduced the incidence of leakage and rupture significantly. The FDA and the manufacturers recommend that the patients obtain an MRI three years after their augmentation and every two years thereafter to make sure implants are intact. MRI is a much more sensitive and accurate test with 89% accuracy in comparison to mammograms that are 65% accurate even on women with no implants. Silicone implants can be

placed via the peri-areolar (around nipple), inframammary (under breast fold) and trans-axillary (armpit) incision. Each has it’s own advantages. At our center, we use small endo-
scopes to perform the trans-axillary approach which can provide a very hidden incision. For all of the women who have undergone breast augmentation at Inland Cosmetic, the procedure has resulted in an aesthetic appearance that has improved their self image and quality of life. The option of Silicone gives our patients another choice that may improve their results even more.

Lipo-Selection vs. Liposuction

Upper Facial Rejuvenation

We often times get patients that come in for eyelid surgery because they complain of “looking tired” all the time or they feel their eyelids have "drooped." The majority of these patients can benefit more from a combination of an Endoscopic Browlift and eyelid surgery. Endoscopic or minimal-scar browlift is performed using a tiny fiberoptic lens inserted through half-inch incisions placed in the hairline. The lens is attached to a

camera, which is connected to a television, allowing for visualization of the surgery. We perform the procedure while viewing the television monitor and manipulating the tiny instruments externally. An endoscopic browlift is particularly useful with younger patients where the removal of excess skin is not required. With an endoscopic browlift, loss of sensation or hair at the incision site is minimized. Since an endoscopic browlift can significantly reduce the

frown lines between the eyes, it is often performed in conjunction with upper eyelid surgery to achieve optimal results. A browlift can be performed as an outpatient procedure under local anesthesia with deep sedation, or general anesthesia, and may take 1 to 2 hours. With this technique, we are able to improve the forehead lines, frown lines as well as raise the brow and improve the entire upper face.

New Fillers On the Market: Instant and Long Lasting Wrinkle Correction

These are exciting times for patients looking for a quick fix for wrinkles on their face. Until recently, collagen was the only choice of filler for wrinkle correction. In the last 3-4 years, the FDA has approved a number of new fillers that last longer and require no prior allergy testing. One such filler is called Restylane.

What is Restylane?
Restylane is a natural, cosmetic filler that adds volume and fullness in order to smooth facial wrinkles and folds. Restylane is made of hyaluronic acid, a natural substance that already exists in the human body.

How long does Restylane last?
The average treatment results last from six to nine months.

Is Restylane safe?
Yes. Restylane has been used safely by dermatologists and cosmetic surgeons worldwide since 1996. Since Restylane is non-animal based and fully biocompatible, allergy testing is not required. The most commonly reported effects are temporary redness and swelling at the injection site, which typically resolves within two to three days. Restylane can be used for correction of the smile lines, wrinkles around the mouth and lips, as well as frown lines.

How is Restylane administered?
Restylane is administered by injection to the site where improvement is desired.

Other Fillers Similar To Restylane: Hylaform, Hylaform PLUS, Captique, Juvederm
These fillers are all made of hyaluronic acid, but with a different consistency which makes them suitable for different areas of the face. For example, we prefer to inject Hylaform in the deep grooves of the lower eyelids, since it is thinner and creates a smoother look than Restylane in that area. Juvederm is the newest filler that has just been approved by the FDA, but has not reached our shelves yet. It should be arriving soon.

What is Sculptra?
Sculptra is another dermal filler that we use in our office for correction of fat loss in the cheeks or midface. Sculptra is different that Restylane because the material injected is gradually replaced by the patient’s own collagen over course of months. Therefore most people will need an average of 2-3 injections of Sculptra on monthly intervals to achieve the improvement that they like. Sculptra lasts 18 months.

How is Restylane different from Botox?

Restylane is a natural cosmetic filler. In contrast, Botox is a temporary paralyzer of muscles that bring about wrinkles. Botox which is one of the most widely used drugs in medicine these days (not just for cosmetic purposes), is more appropriate for treatment of wrinkles in the upper face such as the forehead lines, frown lines and crow’s feet region.

Fillers can be injected into any deep wrinkle on the face, but are especially useful around the mouth and lip areas. Botox and fillers are not mutually exclusive, in fact, we use them in combination very often to improve deep lines and wrinkles on the face.


Accredited Fellowship Program

Maintaining and improving the quality of advanced education in nationally recognized cosmetic surgery fellowships is a primary aim of the American Academy of Cosmetic Surgery (Academy). The Academy is an accredited council of professionals devoted to postgraduate medical education in the field of cosmetic surgery. It is the nation's largest multidisciplinary medical organization that exclusively devotes its educational efforts to cosmetic surgery. Our current fellow is Dr. Mashhadian. Dr. Mashhadian graduated from Mount Sinai School of Medicine and then went on to complete his General Surgery, and Maxillo-Facial Surgery training at Mount Sinai Medical Center.

Previously he had graduated from New York University at the top of his class and earned numerous awards, both in academic and clinical categories. He has joined our practice to complete post-graduate clinical fellowship training in General Body and Facial Cosmetic Surgery accredited through American Academy of Cosmetic Surgery. Dr. Mashhadian’s training in General Surgery, Maxillo-Facial Surgery, Anesthesia, and Cosmetic Surgery are an excellent foundation from which will allow him to succeed and excel in private practice. He is very enthusiastic to have an opportunity to meet with each and every one of you.

Dr. Farzaneh graduated from The College of William and Mary where he earned his Bachelor of Science degree in Biology and chemistry. He obtained his Medical Degree from the University of Maryland School of Medicine. Dr. Farzaneh served his Internship in General Surgery (awarded intern of the year) and his Maxillofacial Surgery Residency at the University of Maryland Hospital Systems and Shock Trauma Center. Dr. Farzaneh served for two years in a fellowship certified by The American Academy of Cosmetic Surgery training in facial and body cosmetic surgery, under the directorship of Dr. Howard Tobin, past president of American Academy of Cosmetic surgery (1994-1995). He trained intensively in breast surgery, body contouring and facial rejuvenation encompassing close to one thousand procedures. He is board eligible in both Cosmetic Surgery and Maxillofacial Surgery.
